HELPFUL TIP:

A deeply embedded fear often stems from trauma, so it can be helpful to explore emotional wounds that may be a factor. Your character might also become risk-averse, which can create problems in their relationships and limit their fulfillment. And for a character to achieve their story goal, they must overcome any fear-based thinking that's holding them back. The Character Builder is a great resource for pulling all these factors together.
NOTES:
Most people have a healthy aversion to failure; it's why we plan ahead, educate ourselves, and weigh options carefully before committing to an opportunity. But a fear of failure becomes problematic when the character is so stymied by it that they limit themselves and become stuck. A character in this situation will need to work through this fear if they're to meet their relational or professional goals and live their fullest life.

WHAT IT LOOKS LIKE:
Being content with the status quo
Taking a follower role; letting others lead
The character not doing their best
Apathy or laziness
Shutting down a co-worker's new idea before it can be adopted
Finding faults in potential love interests
Ending a romantic relationship when it starts to get serious
Turning down new projects or opportunities
Doing something reckless or ill-advised the night before an important test or interview—e.g., staying up all night drinking, then falling asleep and missing the appointment
Procrastinating on a school or work assignment

Blaming others when a failure occurs
Projecting an image that encourages low expectations from others
